Austin ISD Forms

Below is a link to the RankOne website that Austin ISD uses for tracking 3 things you need to have squared away.

These forms must all be taken care of for your son to participate in football activities beginning in August.

1. UIL Forms
2. Physical and Medical History Form

If you have already created an account, you can log in to check your son’s status.  Don’t know if you have any account? On the log in page, use the option for “Continue with One Time Login”.  It will send a code to your email if there is an account associated with it.
 
1. UIL Forms
You have to click through to view 9 documents. Then you can submit the required info. 
 
2. Physical and Medical History Form
You should have already provided this to the coach. It’s the double sided paper from that was completed by a doctor.  If you have not provided it to the coaches, you can upload it on the site.  

If you have not gotten a physical…do it!  This is the form to take to the doctor.

3. Contact Info Form
Just got to fill this one out. Not too much to it.
TIPS:

 

1. Be ready with your son’s Austin ISD S#.

 

2. Do it on a computer; not your phone.

 

3. Have your son with you. Forms require parent and student signatures.

 

4. Although you can complete the forms as a guest, create an account so you can log back in and see your son’s status.

 

5. It’s not the most easy to use site. Be persistent and have some patience.
