Physicals

Every player will need to submit a physical form completed by a medical provider.

Forms are due to the coaching staff by July 21.

Because of a transition with our training staff, the program was not able to offer physicals done at Austin High.

Ways to get your form:

Your son has already brought home a pink form.
Your son can pick up a pink form from the coaches while at Performance Course at Austin High next week
You can download a form from the Austin ISD site here

One option to consider for getting your physical:

CareNow Urgent Care clinics will do physicals for $35 and can be done on a walk-in basis (but probably faster to get in and out with an appointment that you can make online).
